This survey created by Thia Triggs, OTR, is designed to be used by Occupational Therapists or Speech Pathologists working in clinics or schools. It can be used in conjunction with your other oral motor assessments tools. When you have children (or adults) who are in need of food interventions you can use this tool to:

• Interview family members about history and family goals.

Thia Triggs, OT, developed this survey over the past 30 years through her experiences of working in both clinic and school settings, with a variety of clients, and around wide-ranging food issues. Using this tool helps provide a much clearer picture of what is or is not happening and helps the family and therapist to target realistic and measurable goals. A checklist format makes this easy and fast for family members to fill out yet gives the therapist precise information.
